Health Form & Waiver
- Every camper must provide a completed Health Form & Waiver PRIOR to participation
- The Health Form & Waiver must be signed by a parent or guardian
- The Health Form & Waiver does not need to be signed by a doctor

Confirmation: Your confirmation will be sent via email. Online registrations will receive an instant confirmation via email. Mailed applications will receive an emailed confirmation at least 1 week prior to camp.

ONE DAY TRAINING CAMP
Camp Features and Description
- Intense instruction and game play headed by Lehigh University Coaches
- Specialized goalie instruction is provided
- Challenging team competition
- Certified Athletic Trainer on staff
Required Equipment for ALL Campers:
- Lacrosse Stick
- Campers must pack their own dinner - food is not provided
- Goalie Equipment
- Court Shoes and socks
- Field shoes and socks
- Shorts
- T-shirt
- Water bottle
- ***Mouth Guard and Goggles are required for participation
Optional/Suggested Equipment for ALL Campers:
- Rain Gear
- Sweatshirt/pants
- Sunscreen

- Parents must park in the parking lot and come to the field to pick up their child.
- Campers will only be released to authorized pick-up persons at the conclusion of each day. ID will be required for those who cannot be identified by sight.
