Important Club Forms


Forms may be printed out and given to respective club presidents or brought to the IM/Club Sports Office in Grace Hall.

Each participant is responsible to complete all appropriate forms PRIOR to participation in a pracitce or contest which include but not limitied to:
  1. Participation Release
  2. Assumption of Risk and Release of Liability
  3. Medical Insurance
  4. Private Vehicle (if you plan on using your car)
    • Copy of Insurance
    • Driver’s License
    • Vehicle Registration

ALL FORMS NEED TO BE FILLED OUT IN PEN.

REQUIRED OF ALL CLUB SPORT TEAMS
Individual Club Information Due prior to first practice
Club Sport Roster Due prior to first practice
Game Schedule Due prior to first game if you intend to compete with other schools
Facility Request Form Due prior to first practice each semester in order to use Lehigh facilities
Equipment Agreement Due prior to the use of any equipment
Advisor Letter Due prior to first practice
Score Report Due after each game or contest
Referee Form Due after a refereed game for them to get paid
Season Results Due at the end of the season
SAFETY OFFICER INFORMATION
Acceptance of Responsibilty  All clubs must identify two safety officers
Injury Report Form  A report needs to be submitted after any injury
Facility / Event Report  A report needs to be submitted each semester

Club Sport Handbook This form provides all participants about the policies and procedures of Club Sports
TRAVEL FORMS
Club Sport Travel Roster Due one week in advance if the team is traveling
Club Sport Travel Itinerary Due one week in advance in addition to the Club Sports Travel Roster 
Request for Reimbursement  Fill out this form before asking for reimbursements 
TRANSPORTATION INFORMATION
If you need to use a university car, van or Levy yellow bus you must fill out a transportation form. CLICK HERE to access the form.
